Which teeth need sealants, and why

Sealants are recommended almost exclusively for molars and premolars, the back teeth used for chewing, because their chewing surfaces have narrow pits and grooves that a toothbrush bristle often cannot fully clean out, letting food and bacteria settle in and start decay.

Front teeth and the smooth sides of back teeth are naturally easier to keep clean and rarely need a sealant. The best time to seal a molar is shortly after it erupts and before it has any decay, which is why the first permanent molars, typically arriving around age six, and the second molars, around age twelve, are the most common candidates. Adults with deep, cavity-prone grooves and no existing decay or fillings on those teeth can also be good candidates.
